Persian Cat Breed
The main difference between a doll face Persian cat and a peke face Persian cat is the shape of their face. A "doll face" Persian cat refers to a traditional Persian with a more moderate facial structure, meaning a longer, more visible nose. Extreme Persian has a significantly flattened face with a very short, pushed-in nose, often considered the "show" standard for the breed, resulting in more pronounced facial features and potential health concerns due to its extreme brachycephaly. Their eyes have more drainage, which can create hair staining without proper care which can contribute to bacteria and yeast on the fur and skin. A doll face Persian has a normal-length nose that is easily visible, whereas an extreme Persian has a very short, pushed-in nose almost completely flattened against the face. Doll faced Persian has a more rounded, balanced facial structure. Due to their extreme facial features, extreme Persians are more prone to breathing difficulties, eye drainage, and potential complications during birthing compared to doll faces. Doll face is considered the original standard of the Persian cat before the variety was selectively bred to have more extreme facial features.
Peke Face
The peke faced Persian cat has extremely flat features. The name originates from their resemblance to Pekingese dogs that have the same genetic mutation which causes the flat face. Flat-faced Persians are more likely to have health problems, such as brachycephalic airway syndrome, which can involve narrowed nostrils and other upper airway abnormalities. They also often have deformed tear ducts that require daily cleaning.

History of Persian Cats
In the late 1950s, a spontaneous mutation in red tabby Persians gave rise to the "peke-faced" Persian, named after the flat-faced Pekingese dog. Breeders took a liking to the look and started breeding towards the peke-face look. The over-accentuation of the breed's characteristics by selective breeding (called extreme- or ultra-typing). The term peke-face has been used to refer to the ultra-typed Persian but it is properly used only to refer to red tabby Persians bearing the mutation. Many fanciers and CFA judges considered the shift in look "a contribution" to the standard of the breed.
Differences Between Doll and Peke
A "peke" has a nose that sits directly between the eyes which can give the appearance of a grumpy face due to the break in the nose. The eye shape is large, spaced apart, large, and round. The nose sits between the eyes. The nose shape is narrow and the nostrils tend to be more stenotic. The head shape is wide. The muzzle tends to be very narrow.
A "doll face" Persian has a more traditional cat facial structure with an extended, sloped nose that sits below the lower eyelid. The eye shape tends to be round. The head is apple-shaped with small, wide noses, and chubby cheeks. The mouth and nose are in front of the eyes, giving the cat a cute, sweet expression.
Brachycephalic Airway Syndrome
Brachycephalic means “short head” or “flat face.” Common health risks associated with flat-face cats include Brachycephalic Airway Syndrome cats with a shortened or flattened face may have upper airway abnormalities like narrowed nostrils, elongated soft palate, narrow trachea, or soft tissue obstructing their airways, such as everted laryngeal saccules. Felines with this syndrome may exhibit signs like noisy breathing, coughing, snoring, panting, and open-mouth breathing. They may also have trouble exercising due to insufficient oxygen.
